Financial Assistance Policy

Cottage Health assists patients who have limited or no means to pay. Our Financial Assistance Program supports discounted or no-cost care for qualifying patients.


Financial assistance is based on income, residency and ineligibility for government or other insurance. To qualify, a patient must have an income below 350 percent of the federal poverty level and must not have Medicare, third-party insurance or qualify for Medi-Cal, Healthy Families or other government programs. To determine eligibility, we will require documentation from the patient.

How to Apply

State and Federal law requires that Cottage Health provide patients with applications for government health care programs if no proof of government or third-party insurance is provided during registration. Applications for financial assistance can be obtained from an admitting representative or by calling 805- 682-7111, Ext. 53688. In addition, an application can be requested from the business office at 805- 879-8900.

Cottage Health is compliant with California State Assembly Bill 774 and the Affordable Care Act and Section 501(r).
