Cottage Community Health Workers (CHWs)

Patient-based CHWs
Cottage Center for Population Health has launched a patient-based Community Health Worker (CHW) program. Traditionally called promotores in the Hispanic/Latino community, CHWs are part of the Hispanic/Latino community and offer trusted support, guidance and understanding in addressing patient chronic conditions. Cottage CHWs partner with nurses, social workers, and patients’ hospital care teams to support Hispanic/Latino and/or monolingual Spanish-speaking patients with congestive heart failure (CHF).
Helping to transition patients from the hospital to home, CHWs support CHF patients with how to live with and manage their chronic condition, health education, and screening and connection to social needs and referral.

Community-based CHWs
In partnership with community-based organizations in Santa Barbara County, Cottage Center for Population Health supports Community-based CHWs to provide outreach and connection to prevention education and resources, typically on a specific health topic (e.g., COVID-19 prevention and education, increasing access to and education about Cottage Virtual Care).
Cottage partners with community organizations and agencies to provide CHW training.
