At the Cottage Neuroscience Institute, each patient requiring acute inpatient care receives a comprehensive discharge plan for a customized Neuro Rehabilitation Program.

Each patient’s medical acuity, functional abilities and disposition are considered for the determination of post-acute care services. This may include a discharge to home with home health therapies, or outpatient therapy, acute inpatient rehabilitation at Cottage Rehabilitation Hospital (CRH) or a skilled nursing facility.
